Warm Hearth at Home is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team on a FT basis. The LPN will deliver high-quality nursing care to patients in their homes, working under the supervision of a Registered Nurse and in accordance with the plan of care, state licensure guidelines, and organizational standards.

We re looking for someone with strong critical thinking skills, a hospital or home health background, and excellent assessment skills. Someone with strong communication skills, the ability to time management, and someone that adapts well to change.

• Provide skilled nursing care to patients in their homes as directed by the RN and physician orders
• Perform clinical procedures such as wound care, catheter care, medication administration, and monitoring vital signs
• Document timely, accurate, and complete patient information in the electronic medical record
• Collaborate with interdisciplinary team members to ensure coordinated care and positive outcomes
• Communicate effectively with patients, families, and caregivers to educate and provide support
• Recognize and report changes in patient condition to the RN or physician promptly
• Maintain compliance with all policies, procedures, and regulatory requirements
• Assist with obtaining and tracking authorizations for services, working with the clinical and administrative teams

• Current LPN license in the state of VA
• Minimum of one (1) year of LPN experience; home health experience preferred
• Valid driver’s license, reliable transportation, and proof of auto insurance
• CPR certification (BLS) required
• Excellent communication, clinical, and organizational skills
• Ability to work independently and manage time effectively

Warm Hearth at Home offers the additional support our clients need to maintain their current living arrangement or navigate the transition to a higher level of care when needed. Service options include care management, medication management, skilled nursing, rehabilitative therapies and home health aides.
