Schedule- Full‑time in‑office position, Monday – Friday with an on‑call rotation.

- Ensure that client complaints, issues and concerns are communicated to the COO and the Administrator.
- Ensure compliance at the assigned location with clinical standards, regulations, and company policies; oversee patient and personnel assignments.
- Oversee the referral process and ensure clients meet admission criteria.
- Ensure continuous client needs assessment and individualized Plan of Care development.
- Ensure vulnerable adult and child protection reports are conducted per company policies.
- Discharge clients per established policies.
- Participate in on‑call process to ensure policies are followed.
- Implement company Performance Improvement Plan (PIP) as directed by the Director of Quality.
- Execute strategies and initiatives to meet operational goals.
- Meet Clinical Manager requirements of Medicare Conditions of Participation.
- Onboard, supervise, evaluate, train, assign, and discharge staff per policies; notify HR of employee issues.
- Mentor and hold staff accountable for performance and goals.
- Ensure ongoing training and performance management per policies.
- Oversee compensation policies and ensure guidelines are met.
- Implement location safety programs and complete Workers’ Compensation reports.
- Fill key roles during temporary vacancies.
- Participate in strategic company growth and performance improvement.
- Recommend sales and growth strategies for the location.
- Implement long‑range plans for expenditures and resources per business plan.
- Develop and maintain relationships with community agencies and referral sources.
- Monitor key financial indicators.
- Track financial performance and key metrics.
- Ensure services follow payer guidelines.
- Submit documentation for reimbursement and monthly reporting.
- Ensure payroll meets policies and guidelines.
- Maintain equipment and supplies in working condition.
- Manage work assignments effectively, remain flexible to changes, and handle additional duties as needed.
Primarily office work requiring frequent sitting, standing, bending, and stretching. Must be able to drive or use public transportation in all weather conditions.
Light physical demand: occasional movement up to 20 pounds.
Qualifications- Registered Nurse (RN) licensed in Minnesota or a graduate of an accredited Physical Therapy program licensed in Minnesota.
- License must not be revoked, suspended, or restricted.
- Proof of negative Tuberculosis testing (Mantoux or chest X‑ray).
- Dependable automobile for multiple daily business stops.
- Valid driver’s license and proof of car insurance when using a personal vehicle.
- U.S. Citizenship or valid Alien Work Permit.
- Disclose any criminal convictions; candidates with convictions will not be hired.
- Pass initial and ongoing background studies and screenings (Minnesota Department of Health, FOI Inspector General Excluded Individuals list).
- Five years of progressive health‑care management experience; home‑health experience preferred.
- Critical thinking, adherence to standards, clinical judgment for safe client care.
- Independent work ability, adaptability, priority setting, and duty completion.
- Excellent written and oral communication, interpersonal and teaching skills.
- Mentoring and leadership development for staff.
- Basic computer skills; willingness to learn new software; experience with EMR and Outlook preferred.
- Current CPR certification highly recommended.
Salary: $100,000 – $118,000 per year plus bonus incentive program. Salary depends on experience, certifications, level, and location.
