- This is a full-time position, Monday - Friday with an on‑call rotation.
- This position requires traveling to client homes for 1:1 client care that may take 1–3 hours per client.

This position reports to the Blaine, MN office and provides services in Blaine and surrounding cities.

- Plan, provide, and evaluate nursing care in accordance with established policies and procedures and consult with the Case Manager regarding any changes in the care plan.
- Demonstrate knowledge of current health practices when performing observation and evaluation of client care.
- Provide total client care to include medications and treatments administered as ordered by the physician, physical and psychological care, and client and family education.
- Work with families and other health‑care professionals to provide continuity of client care.
- Assess and interpret symptoms and institute remedial measures within the legal limits of nursing practice within the state of Minnesota, reporting significant changes in client’s condition to the Case Manager immediately.
- Exercise professional judgment in adapting nursing measures to changing needs.
- Communicate effectively with all members of the interdisciplinary team.
- Participate in scheduled client conferences as requested.
- Follow infection control policies, including proper hand washing techniques.
- Maintain accurate clinical record, preparing clinical and progress notes with timely record keeping.
- Take responsibility for professional growth and development by actively participating in continuing education, in‑service programs, orientation, and personnel evaluations.
- Maintain absolute confidentiality of all information pertaining to clients, families, and employees.
- Maintain a safe client environment and identify and report to the office any suspected vulnerable client abuse, neglect, or financial exploitation.
- Promote a positive client and employee service image.
- Exercise judgment in accepting assignments.
- Participate in the after‑hours on‑call process to ensure client care and staffing issues are resolved.
- Perform other related duties and responsibilities as assigned.
Heavy to Very Heavy. Heavy lifting necessary for client care. Nursing care provided in individual client homes. Must be adaptable to a variety of environments and community settings. Must be able to drive or use public transportation in all types of weather. Exertion may exceed 100 pounds of force occasionally, 50 pounds frequently, or 20 pounds constantly to move objects.
Qualifications- Currently licensed as an R.N. in the state of Minnesota. The license must not have been revoked, suspended, or contain restrictions.
- Have at least two (2) years’ experience in a health‑care setting.
- Acute hospital or home‑care experience preferred.
- Demonstrate ability to make appropriate nursing judgments.
- Possess good interpersonal communication and teaching skills.
- Good organizational skills and work independently.
- Excellent written and oral communication skills.
- Proof of negative Mantoux or documentation of negative chest x‑ray.
- Current CPR certification highly recommended.
- Access to a dependable vehicle or public transportation to travel to multiple company stops a day.
- Valid driver’s license and proof of car insurance when using a personal vehicle for company business.
- Have U.S. citizenship or evidence of a valid Alien work permit.
- Disclose any conviction and criminal history records pertaining to any crime related to the provision of health services. Candidates convicted of such crimes will not be hired.
- Pass initial and ongoing background studies and screenings, including those of the Minnesota Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals and Entities.
Adara Home Health, Inc. is committed to fair and equitable compensation practices. Base salary range for this position: $70,000 – $85,000 per year. Salary is determined by relevant factors including experience, skills, certifications, job level, and location.
