Shift: Replacement for Juliette Nedrick. Monday through Friday 8‑4:30 pm, weekends and holiday rotation

• Provide personal care and housekeeping assistance to clients in their homes; follow the established plan of care under the direction of the client care coordinator
• Provide personal hygiene and grooming care to clients
• Provide meals to meet clients’ nutritional needs
• Report changes in clients’ condition to the client care coordinator or supervisor
• Perform routine housekeeping and home management duties
• Record each client’s care on the care plan and submit the plan in a timely manner
• Respond sensitively to the multiple, complex needs of terminal clients, including providing end‑of‑life care
• Assist with routine therapy exercises

• Education:
  
  High school diploma or equivalent (preferred)
• Licensure/Certification: CNA Certificate approved by the Connecticut Department of Public Health and Addiction Services (required)
• Experience:
  
  Six months experience as a Home Care Aide/CNA in a home care agency (preferred)
