The Region Clinical Intake Specialist is responsible for managing and coordinating the clinical intake process for both Home Health and Hospice services across multiple branches. This role ensures timely, accurate, and compliant intake procedures using the Homecare Homebase (HCHB) system, serving as a key liaison between referral sources, clinical teams, and operational leadership.
Perform all clinical aspects of the intake process, including communication with patients, families, and admitting clinicians; ensure clinical data entry accuracy, timeliness, and Face‑to‑Face encounter accuracy; verify PCP/MDs; and ensure patients are admitted in accordance with intake policy. Provide recommendations for process and policy improvements and ensure compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.
Stay thoroughly informed about the details of clinical quality metrics and collaborate with Home Care Leadership, healthcare networks, and other partners for Home Health and Hospice services. Ensure patient admissions comply with contract terms and maintain positive relationships with referral sources and clinical leadership teams.
Coordinate care for patients by overseeing education, care planning, and coordination with multiple providers, case managers, and other clinical needs within established payer and financial guidelines. Strengthen and monitor community and customer perceptions of Hartford Healthcare at Home as a high‑quality service provider while collecting and reporting clinical referral data and key customer trends.

QualificationsEducation: Licensed clinician (RN, LPN, or equivalent) required. Bachelor’s Degree (BSN, Healthcare Administration, or related field) preferred.
Experience: 1 year of administrative/clinical healthcare experience; 1‑3 years of experience preferably in Home Care.
Knowledge, Skills and Abilities: Healthcare experience in referrals/intake in a home health or hospice environment; good communication, negotiation, and public relations skills; autonomy, assertiveness, flexibility, and cooperation in performing job responsibilities.
