Wound Care LPN – Home Care

Hartford Health Care at Home, Connecticut’s largest provider of home‑care services, is seeking a licensed practical nurse to join our Wound Care Program. As an LPN, you will partner with the Wound Team RN, CWOCN, and manager to deliver high‑quality wound care in clients’ homes and help achieve excellent clinical and operational outcomes.

• Deliver wound care and related medical/surgical services to meet patient needs.
• Collaborate with the Wound Team RN, CWOCN, and physician/wound clinics to adjust care plans for complex wounds.
• Assist with staffing, training, and onboarding of new staff to the wound program.
• Participate in the interdisciplinary approach to client care and support clinical education initiatives.
• Provide consultation to the Wound Team RN and CWOCN on patient issues requiring changes in interventions.
• Monitor and report on clinical and operational performance measures; contribute to quality improvement efforts such as H3W.
• Research and communicate new wound‑care information and recommend treatment options to enhance program standards.
• Perform other duties as assigned that support the wound program’s success.

• CT‑approved Licensed Practical Nursing program required.
• Wound/Ostomy Assistant certification preferred; required to obtain within 6 months of hire.
• Minimum of two years of nursing experience in a clinical specialty area; home‑care experience preferred.
• Valid LPN licensure in Connecticut.
