We currently have clinical opportunities to join an experienced multi-disciplinary team of clinicians providing Hospice and Palliative care.
Job Description:
RN Hospice Home Care. Provide hospice and palliative care at home, working either Friday, Saturday, Sunday or Saturday, Sunday, Monday for a 24‑hour paid position of 40 hours. Ensure compassionate delivery of care to patients in their final journey. Work with an interdisciplinary team of social workers and pastoral counselors to provide families and patients with understanding and support during this process.

Qualifications
- Licensure:
Registered Nurse with a license in the State of Connecticut - Education:
Graduate of an Associate’s or diploma program in nursing; graduate of NLN approved BSN program (preferred); OASIS certification - Experience:
One year nursing experience or one year nursing experience in a specialty (medical/surgery or home care) - Computer skills preferred
