We are seeking compassionate, dependable, and experienced Live-In Licensed Practical Nurse (LPNs) to provide exceptional one-on-one care in a client's home. Candidates must have a strong skilled nursing background and be comfortable assisting with wound care, and providing high-quality personal care.

Responsibilities

• Provide compassionate live-in care and supervision.
• Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, transfers, and mobility.
• Monitor and report changes in the client's condition.
• Assist with medication reminders and prescribed care plans.
• Perform wound care according to the physician's orders and agency protocols (required experience).
• Administer daily injections as prescribed (LPNs only).
• Maintain accurate documentation of care provided.
• Prepare light meals and assist with feeding if needed.
• Perform light housekeeping related to the client's care.
• Communicate effectively with family members, physicians, and the care team.
• Ensure a safe, clean, and comfortable environment for the client.

Qualifications

• Active LPN license in good standing.
• Skilled nursing experience required.
• Experience with wound care management.
• Ability to administer daily injections safely and accurately.
• Current CPR certification.
• Excellent clinical judgment and communication skills.

Preferred Qualifications

• Experience in hospice, rehabilitation, or long-term care.
• Experience with diabetic care, chronic wound management, and post-surgical recovery.
• Ability to work independently in a home setting.
