Position: Licensed Practical Nurse (LPN)
Setting: Skilled Nursing Facility (SNF)
Shift: 5:00 PM – 5:30 AM (12.5-hour shift)
The Licensed Practical Nurse (LPN) provides direct patient care in accordance with established policies, procedures, and standards of practice within a Skilled Nursing Facility (SNF). The LPN works under the supervision of a Registered Nurse (RN) and collaborates with the interdisciplinary care team to ensure high-quality resident care.
Key Responsibilities- Administer medications and treatments as prescribed.
- Monitor and document resident health status, reporting changes promptly.
- Assist with admissions, transfers, and discharges.
- Provide wound care, catheter care, and other nursing interventions.
- Supervise and support Certified Nursing Assistants (CNAs).
- Ensure compliance with infection control and safety protocols.
- Maintain accurate and timely documentation in resident records.
- Current and valid LPN license (state-specific).
- CPR/BLS certification required.
- Prior experience in long-term care or SNF preferred.
- Strong communication and organizational skills.
