LVN​/LPN – LTC​/SNF

Job Summary:

Provide skilled nursing care to residents in a Long-Term Care/SNF setting, ensuring high-quality patient outcomes. Deliver compassionate, professional support while adhering to regulatory standards and facility protocols.

Responsibilities:

• Administer medication and treatments accurately and efficiently.
• Monitor residents’ health status and document changes promptly.
• Collaborate with the healthcare team to develop and implement care plans.
• Adhere to infection control and safety procedures at all times.
• Educate residents and family members on health management and care routines.

Qualifications:

• Valid LVN/LPN license for the state of practice.
• Proven experience in Long-Term Care or SNF environments.
• Strong assessment and clinical skills.
• Ability to work flexible hours, including weekends if required.
• Knowledge of state and federal healthcare regulations.
• Excellent communication and interpersonal skills.
