Responsibilities- Responsible for performing the daily activity calendar requirements set by the Activity Director. Support and follow the Activity Program’s mission, philosophy, policies and procedures and state and federal regulations. Aim to achieve and maintain the highest quality of life for each resident, with understanding of psychosocial, medical, and activity needs/preferences.
- Lead a therapeutic, person-centered activity program that enhances the quality of life for residents and aligns with the Activity Mission and Philosophy.
- Follow all Activity Policies and Procedures and Federal and State regulations.
- Each resident’s background, culture, life history, disease and medical care needs should be understood to ensure appropriate person-centered activity plans.
- Schedule and conduct daily activities that reflect residents’ schedules, choices, and rights.
- Communicate with residents and families and coordinate with other health professionals and community providers as needed.
- Document and complete required documentation per the Activity Director and maintain necessary records.
- Assist in updating activity boards and calendars with a monthly theme that is creative, neat, and clean.
- Facilitate community involvement and assist in resident outings, including coordination of transportation when applicable.
- Maintain the activity schedule and equipment in good, clean order and notify the Activity Director when supplies or repairs are needed.
- Perform other duties as assigned.
- High school education or equivalent.
- At least 1 year of experience in the healthcare field, volunteering, or personal experience with the disabled population.
- Current knowledge of Federal and State activity regulations and/or ability to understand and interpret.
- Ability to read and write English.
- Bi-lingual abilities may be preferred depending on the facility.
- Valid IL driver’s license.
- Punctual and time efficient, especially in following the activity calendar.
- Empathy, compassion, understanding, enthusiasm, personable, energetic, positive attitude, and a sense of humor to interact with residents, families, and staff.
- Ability to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile individuals.
- Effective communication with residents, families, health professionals, and community providers.
- Ability to manage emotional needs of yourself and staff.
- Must be able to push, pull or carry 50 pounds.
- May be required to lift equipment, supplies, and occasionally move furniture.
- Must be able to move intermittently throughout the workday.
- Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, family members, personnel, and support.
- Provide feedback to the Activity Director for monitoring goal achievement through a team approach.
- Lead a therapeutic, person-centered activity program that enhances residents’ quality of life.
- Follow all Activity policies, procedures and Federal/State regulations.
- Know each resident’s background and needs to ensure appropriate activity plans.
- Ensure activities are daily and reflect residents’ rights and choices.
- Understand communication needs of residents, including language services and communication boards.
- Conduct and document ongoing assessments related to social, medical, cognitive, communication, spiritual, physical, and behavioral needs as they relate to activities.
- Document required information per the Activity Director and maintain records such as Resident Daily Participation, Progress notes, Leisure Interest Survey, and Activity Planning sheets.
- Update calendars and boards daily with creative, neat, and clean themes.
- Engage community involvement and assist in resident outings; may include driving residents as needed.
- Follow Activity staff schedules with a team approach and continuity of care.
- Work as a team with staff and volunteers; avoid canceling activities without permission.
- Maintain equipment and supplies; notify the Activity Director if materials are needed or repairs are required.
