Licensed Practical Nurse (LPN) – The Alden Network
Location:
South Elgin, IL
Responsible to provide direct nursing care to the customer, and to supervise the day‑to‑day nursing activities performed by the nursing assistants. Such supervision must be in accordance with current Federal, State, and local standards, guidelines and regulations, facility policies. The objective is to ensure the highest degree of quality care is maintained at all times.
Responsibilities- Provide direct nursing care to the customer.
- Supervise day‑to‑day nursing activities of nursing assistants.
- Ensure compliance with federal, state, and local standards, guidelines, regulations, and facility policies.
- Perform nursing procedures and protocols following established policies.
- Directly supervise and instruct staff on job knowledge and departmental procedures.
- Develop written care plans identifying problems/needs of each customer.
- Attend and actively participate in customer care plan conferences.
- Report concerns, complaints from customers or families to the Administrator.
- Provide counseling/discipline to nursing personnel as requested.
- Conduct daily rounds to ensure nursing personnel perform duties and procedures correctly.
- Create a warm, positive, calm environment within the department.
- Participate in continuing education programs.
- Recommend equipment and supply needs to the Director of Nursing.
- Place orders for medications and treatments following budgetary guidelines.
- Utilize PCC 24‑hour summary concerning customer care needs.
- Complete accident/incident reports.
- Encourage physicians to review treatment plans and record orders.
- Chart nursing notes accurately reflecting care provided.
- Complete required assessments upon admission, transfer, and discharge.
- Receive telephone orders from physicians and record them.
- Record new/changed diet orders and forward information to the Dietary Department.
- Perform routine charting duties per established policies.
- Complete admission, discharge, and transfer processes.
- Administer medications and treatments as ordered by physicians.
- Review medication records for accuracy and policy compliance.
- Administer professional services such as catheterization, tube feedings, suction, dressing changes, colostomy care, etc.
- Orient new customers/family members to the facility.
- Arrange diagnostic and therapeutic services as ordered by physicians.
- Notify physicians and families of changes in customer conditions.
- Obtain sputum, urine, and other lab tests as ordered.
- Take and record vital signs, blood pressures, etc.
- Contact physicians for assessment, medication orders, incident reporting, lab results, or other matters.
- Ensure nursing notes and summaries reflect the care plan.
- Attend staff meetings as directed.
- Perform other related duties as assigned.
- Current, unencumbered license to practice as an RN or LPN in Illinois or Wisconsin.
- Current CPR certificate or be certified within two weeks of employment.
- Knowledge of nursing and medical practices and procedures.
- Interest and/or knowledge in caring for the aged.
- Able to read, write, and speak English understandably.
- Independent decision‑making ability when warranted.
- Ability to deal tactfully with customers, families, visitors, personnel, government agencies, and the general public.
- Leadership and supervisory ability and willingness to work harmoniously with other personnel.
- Personable, enthusiastic, and resourceful.
- Move intermittently throughout the workday.
- Cope with the mental and emotional stress of the position.
- Possess sight/hearing or appropriate prosthetics to meet job requirements.
- Function independently, with flexibility, personal integrity, and ability to work effectively with customers, families, personnel, and agencies.
- Maintain good general health and emotional stability.
- Relate to and work with ill, disabled, elderly, emotionally upset, and occasionally hostile people.
- May be required to lift, push, pull, and move equipment, supplies, and customers in excess of fifty (50) pounds.
- Subject to exposure to infectious waste, diseases, conditions—including HIV, AIDS, and Hepatitis B viruses.
