Pay range:

• $35 – $40 / hour

Elite Care Management is a privately owned home health care company with over 25 years of experience in providing compassionate, high-acuity care. We specialize in supporting adult patients with Spinal Cord Injuries, Traumatic Brain Injuries, and other complex medical needs.

As a Skilled Nurse (RN or LPN), you will provide one-on-one care in a patient’s home. This unique setting allows you to develop meaningful relationships with patients and their families, delivering personalized, uninterrupted care throughout your shift.

Responsibilities may include the following:

• Observe patients and report significant conditions, reactions to medications, or changes in health status to the supervisor or physician
• Understand and follow all nursing policies and procedures
• Communicate clearly and professionally, both verbally and in writing
• Deliver patient care based on the established care plan, updating as necessary
• Administer medications and treatments as prescribed, monitoring their effectiveness
• Respond promptly and appropriately to changes in patient condition
