We provide fully integrated care for all our patients by combining behavioral health care services with our community-based primary care health centers.

• Conduct mental health, domestic violence, and substance use evaluations with clients and families.
• Maintain current and complete documentation in client’s electronic medical record.
• Collaborate with referral sources and internal teams to coordinate client services.
• Provide personalized recommendations and care plans.
• Maintain regular communication with client support systems such as probation officers, family members, and psychiatrists, etc.
• Testify in court when required to advocate for clients.
• Engage in ongoing professional development and team meetings.
• When requested assist with training of new clinicians and interns.
• Uphold Chestnut's commitment to customer service excellence and confidentiality.

• Bachelor's degree in a human services-related field with five years of supervised mental health or substance use disorder experience
  OR
• Bachelor's degree with current Certified Alcohol and Drug Abuse Counselor (CADC) certification
• Must obtain CADC certification within two years of hire if not already certified.
• Valid driver's license, private auto insurance, and insurability

The anticipated starting pay for new hires for this position is between $21.00 - $22.00 an hour. There are several factors taken into consideration in determining base pay, including but not limited to: job-related qualifications, skills, education, experience, local market conditions, and internal equity.
