Position: Case Manager (Housing)

Overview

Chestnut Health Systems™ is seeking a compassionate and detail-oriented Case Manager (Housing) to join our team in Bloomington, Illinois
. This position plays a vital role in supporting individuals on their path to recovery and stability by providing housing-focused case management services, fostering independence, and promoting wellness. The Case Manager will deliver client-centered support using trauma-informed and recovery-oriented care principles.

We provide fully integrated care for all our patients by combining behavioral health care services with our community-based primary care health centers.

Responsibilities
  • Complete all required documentation and data entry in accordance with agency and program standards, including but not limited to LOCUS, IM-CANS, HRA/PHA, and progress notes. Ensure 100% of IM-CANS are completed prior to due dates. Submit progress notes and other required paperwork within 48 hours of service delivery.
  • Provide services identified in the IM-CANS to meet consumer needs and program requirements, including: maintaining required service levels and timely completion of authorizations; delivering clinical services such as community support, case management, therapy, and medication training; supporting a clean, safe, and healthy living environment for residents; providing services in the most effective setting, including consumers’ apartments or community locations; linking consumers to additional clinical and community resources as needed;

    completing clinical screenings of new tenant housing referrals when assigned.
  • Promote the recovery model of treatment by assisting consumers in achieving self‑directed recovery and incorporating trauma‑informed care principles such as hope, empowerment, and respect.
  • Maintain positive relationships within Chestnut and with community partners including hospitals, courts, medical offices, and social service agencies to ensure coordinated, high‑quality care.
  • Complete and maintain accurate documentation to meet housing funder requirements, including attendance and census tracking.
  • Provide referrals, advocacy, and linkage to essential community resources such as healthcare, food, financial aid, and housing supports.
  • Participate in meetings, case staffings, and program development activities. Provide backup and support to other team members as needed. Attend 100% of required supervision (Rule 132) if not an LPHA. Complete all mandatory Chestnut trainings and continuing education requirements through Relias.
  • Utilize evidence‑based practices focused on skill‑building, therapy, and recovery support for individuals, families, and groups.
  • Take a leadership role on assigned projects and initiatives.
  • Uphold Chestnut’s culture of customer service excellence by adhering to organizational behavioral standards.
  • Protect the confidentiality of all client and organizational information at all times.
Qualifications
  • Education and Experience:

    Bachelor’s degree in a behavioral health field; or Bachelor’s degree in an unrelated field with 2+ years of behavioral health experience; or High school diploma or equivalent with 5 years of social service or peer experience; or High school diploma or equivalent with CRSS or CPRS certification (in good standing with the State of Illinois).
  • Proficient in basic computer applications including Microsoft Word and Electronic Medical Records (EMR).
  • Valid driver’s license, private automobile insurance, and insurability required.
  • Strong written and verbal communication skills.
  • Ability to work collaboratively with a multidisciplinary team.
  • Demonstrated understanding of trauma‑informed and recovery‑oriented practices.
Pay

The anticipated starting pay for new hires for this position is between $21.00 - $22.00 an hour.
