The RN is responsible for providing high‑quality care in a collaborative, team‑based environment.

• Assess residents and develop individualized care plans in collaboration with the interdisciplinary team (IDT).
• Coordinate with IDT to optimize health outcomes.
• Supervise CNAs and ensure compliance with Illinois Department of Public Health / Centers for Medicare & Medicaid Services (IDPH/CMS) standards.

• Active Illinois Registered Nurse license.
• Long‑term‑care (LTC) experience preferred.
• Strong communication and leadership skills.
