Day shift (9a-9p), 12-hour shifts, rotating weekends and holidays Pay Range: $38.20 - $57.30 Registered Nurse (RN) Advocate Health | Hospital at Home Program Park Ridge, IL (Lutheran General Hospital) Full Time; 12 hour day shift Sign on Bonus $5,000 paid out 50% at 90 days, 50% at 1 year of employment (External Hire Only)

- Manageable Caseload: 3-4 patient assignments, which may include admits/discharges, per 12-hour shift

Schedule:
Includes alternating weekends - Competitive Hourly Pay
- Deliver acute, hospital-level care in patients’ homes
- Perform skilled clinical tasks (IV starts, phlebotomy, medication administration)
- Monitor patient conditions and recognize changes requiring escalation
- Collaborate with a virtual and in-person interdisciplinary team
- Use evidence-based practice to ensure safe, patient-centered care
- Coordinate discharge planning and ongoing care needs
- Promote a culture of safety and high-quality outcomes
- Active RN license in the state of Illinois
- BLS certification (AHA)
- Valid driver’s license with a clean driving record
- 2+ years of recent acute inpatient care or ED experience preferred
- Strong IV, phlebotomy, and clinical assessment skills
- Ability to work independently in home settings
- Excellent communication and critical thinking
- Comfortable with technology and remote monitoring tools
- Work Environment Travel within designated service area
- Provide care in private home settings
- Physical activity required (lifting, bending, standing, etc.)
