- Collaborate with healthcare team to develop and implement patient care plans.
- Educate patients and families on post‑discharge care and health management.
- Maintain accurate documentation and ensure compliance with hospital protocols.
- Utilize critical‑thinking skills to respond to patient needs promptly and effectively.
