Position Responsibilities
- Complete patient assessments and develop individualized plans of care.
- Coordinate and oversee patient care to ensure quality and continuity of services.
- Monitor patient progress and communicate changes in condition to the appropriate healthcare professionals.
- Conduct scheduled home visits throughout Bloomington and surrounding areas.
- Collaborate with physicians, patients, families, and the care team.
- Maintain accurate, timely clinical documentation in accordance with agency policies and applicable regulations.
- Educate patients and caregivers regarding care plans, medications, and health-related needs.
- Help ensure services are delivered in accordance with established clinical standards and home health requirements.
Qualifications
- Current, unrestricted Registered Nurse (RN) license in Indiana.
- Strong communication, organization, and time-management skills.
- Compassionate, dependable, and patient-focused approach to care.
- Ability to work independently while collaborating with a multidisciplinary team.
- Valid driver’s license and ability to meet company vehicle insurance requirements.
- Home health experience and case management experience preferred.

In-home patient visits - Office reporting:
Monday-Friday at 8:00 AM to pick up the company vehicle before completing scheduled visits.
