*** About the Role
** Select Home Health is seeking a motivated and compassionate Licensed Practical Nurse (LPN) / Account Executive to provide high-quality in-home nursing care while building relationships with healthcare providers and referral sources. This role combines hands-on patient care with sales, community outreach and referral development to support patient well-being and grow our home health services.
** Responsibilities**
* ** Clinical Nursing Responsibilities
*** Provide skilled nursing care in accordance with physician orders and established care plans.
* Monitor patient conditions, assess pain and educate patients and families on care needs.
* Administer medications safely and follow established medication administration procedures.
* Promote patient safety, confidentiality and quality of care.
* ** Sales & Referral Development
*** Build and maintain relationships with healthcare providers, community partners and referral sources.
* Educate referral partners about Select Home Health services.
* Identify opportunities to expand referral networks and increase patient census.
* Execute outreach plans and track activities in the CRM.
* Represent Select Home Health at community events and networking opportunities.
* Monitor market trends and identify opportunities for growth.
* ** Administrative & Team Collaboration
*** Maintain accurate records and complete required reports.
* Coordinate with clinical teams to support patient needs and referral transitions.
* Collaborate with leadership on outreach initiatives and agency growth.
* Ensure clinical and business development activities comply with applicable regulations and agency policies.
* Communicate patient status and needs with the interdisciplinary care team.
* Complete accurate and timely clinical documentation.
* Participate in required trainings, in-services and meetings.
* Follow agency policies, procedures and emergency protocols.

* Graduate of an accredited nursing program
* Active state LPN license in the state of Indiana
* Recent acute care or home health experience (home health preferred)
* Valid driver’s license, reliable vehicle, and auto insurance
* Current TB test and CPR certification
