Agency:
Direct Care and Treatment (DCT). The SDS serves as the lead worker at the program, overseeing programs, activities, and scheduling under supervision. The SDS splits time between direct care responsibilities and programmatic responsibilities to design, implement, and evaluate behavioral support strategies for individuals at the site. Hours vary; candidates must be available to work evenings and weekends as needed. This role is suitable for candidates with experience supporting individuals with high behavioral needs.

Primary

Responsibilities
  • Participate in the development, refinement and implementation of individualized program plans to help individuals safely achieve their goals.
  • Provide work direction, guidance, training and feedback to staff to ensure high standards of service delivery.
  • Provide direct care to individuals (bathing, toileting, hygiene, medication/treatment administration, escorts and mobile supports, and behavior interventions).
  • Contribute to the safety and security of the site, individuals served, and co-workers.
Minimum Qualifications

Must meet the qualifications to be a Designated Coordinator:

  • Option A: 4 years of full-time direct care experience with persons with disabilities or persons age 65+
  • Option A continued: 50 hours of education/training related to human services and disabilities

Or one of the following related to human services with required experience:

  • Diploma in a field related to human services from an accredited post-secondary institution and 3 years full-time direct care experience
  • Associate degree in a field related to human services and 2 years full-time direct care experience
  • Bachelor’s degree in a field related to human services and 1 year full-time direct care experience

Human Services related degrees may include, but are not limited to:
Family Social Sciences, Health Care Management, Special Education, Public Health, Recreational Therapy, Social Work, Alcohol and Drug Counseling, Social Science, Health Science, Psychology.

  • Must have a valid Driver’s License
  • Must be at least 21 years of age
Abilities, Knowledge and Skills
  • Develop person-centered support plans using Positive Support concepts (Positive Behavior Support, Person-Centered Thinking and Planning, recovery-oriented supports).
  • Communicate effectively, both orally and in writing, to promote shared understanding of goals and implementation.
  • Develop activity schedules to ensure efficient use of resources.
  • Assess interfering behavior, skill development needs, and progress toward goals using consistent principles.
  • Work rotating or irregular hours as needed; provide direct care as required.
  • Knowledge of coaching principles to motivate employees and support plan design/implementation.
Preferred Qualifications
  • Experience with culturally responsive work and diverse backgrounds.
  • Lead work experience in a direct care setting.
  • Experience writing, modifying, or evaluating program plans.
  • Experience with financial management of home or individual accounts.
