Join our team as an LPN (Licensed Practical Nurse) at Great Lakes Management. LPN certification in the state of MN is required.

$28.00/hr - $30.00/hr

• Ensure residents maintain admission criteria, monitor, and recommend alternative placement if necessary.
• Ensure residents have up-to-date care plans per state regulations.
• Monitor and maintain resident charts and clinical progress notes.
• Communicate to physicians, staff, and family changes in residents’ needs or conditions.
• Facilitate coordination of resident services with other service providers.
• Oversee medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Ensure medication administration procedures are followed properly and discuss any needed corrections, errors, etc., with staff.
• Record any information staff need to be aware of in the resident’s log.
• Provide alternating supervisory visits in coordination with the RN.
• Work as a team player with all members of the residents’ care team.
• Supervise medication practices and medical equipment being used in the building.
• Ensure staff clearly understand and follow all resident care procedures.
• Share resident and family concerns with pertinent staff when appropriate.
• Work with manager to assure emergency procedures are in place.

• Maintains a current state license as an LPN (Licensed Practical Nurse) per state regulations.
• A minimum of one year’s experience in home health, assisted living or long‑term care is preferred.
• Good organizational skills with the ability to self‑start and follow through with initiatives with minimal supervision.
• Exhibits a caring heart with a passion to care for the elderly.

Full time, including every other weekend.
