• Valid LPN license in PA
• CPR/BLS certification
• Experience in long‑term care, rehab, or hospital settings is a plus
• Clear background and up-to-date health screenings (Physical and TB)

• Administer medications, wound care, and treatments under RN or physician supervision
• Monitor and document patient conditions
• Communicate with families and medical teams to ensure care plans are followed
• Provide leadership and support to CNAs and other care staff
