Housing Case Manager
Central Susquehanna Opportunities, Inc., an innovative non‑profit Community Action Agency, is hiring a full‑time ESG Housing Case Manager to work in Columbia/Montour Counties.
The ESG program helps people quickly regain stability in permanent housing after experiencing a housing crisis and/or homelessness.
Responsibilities- Provide intensive case management services, including assessment of housing barriers and needs.
- Develop action plans and goals to remove barriers.
- Locate and maintain suitable housing for clients.
- Conduct landlord outreach.
- Coordinate services and referrals with community agencies.
- Engage in community outreach.
- Bachelor’s Degree in a related field preferred (or a combination of education and experience that demonstrates the required skills, knowledge and abilities).
- Experience in case management or related field.
