** What You Will Do as a Hospice RN Field Nurse

As a Hospice RN Field Nurse, you will provide support to patients and families to ensure a safe and comfortable environment.

Some of your primary responsibilities will include:
* Completing an initial assessment of patient and family to determine hospice care needs. Providing a complete physical assessment and history of current and previous illness
* Developing a care plan, establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative actions
* Re-evaluating and revising patient care needs as needed
* Administering medications and treatment safely and as prescribed by the physician
* Counseling and educating patients and families in meeting nursing and related needs
* Providing health care instructions to the patient as appropriate per assessment and plan of care
* Participating in on-calls

** Education, Licensure & Certification Requirements
*** Graduate of an accredited school of nursing
* Active RN license
* One (1) to two (2) years of recent acute care experience
* Bachelor’s degree with one (1) year of hospice/home health experience preferred
