Sage Group Health Care  Staffing is seeking Hospice RN Case Manager for a travel/contract position in Blue Ash, OH. Accepting both local and traveler candidates.

Pay Rate: $2000 Weekly Gross pay based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments

Shift timings: 8-hour Days, 8.00 am to 5.00 pm

Schedule: 40 hours/week guaranteed. 5 X 8-hrs, 5 shifts per week, Mon-Fri. On-call every other weekend from Friday, 5 pm through Monday, 8 am

• Registered Nurse: 2years
  - Required
• Recent/Current Hospice nursing experience in a home health setting: 1 year within the last 3 years – Required
• Home Care Home Base(HCHB) charting experience – Required
• Must have valid
  
  Driver’s License and Personal Auto Insurance
• Must be Covid Vaccinatedor provide medical or religious exemption

• ASN or an equivalent– Required
• BSN or an equivalent– Highly preferred
• RN License for the Stateof Ohio or Compact
  - Required
• BLS Certification from AHA or American Red Cross
  - Required

• Provide and coordinate care for assigned Hospice patients and manage assigned caseload
• Develop and implement nursing care plans for patients as assigned.
• Empower patients and families to reach healthcare goals by educating them on disease management.
• Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
• Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
• Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
• Notify physicians of significant changes in status.
• Document and complete charting
• Territories:
  Area of coverage for Mon-Fri is the Greater Cincinnati area for M-F. For every other weekend, on-call will cover Cincinnati and Dayton offices
