• Provide 1:1 care following client care plans while maintaining dignity and independence
• Administer medications, feedings, oxygen, and ostomy care as needed
• Accurately document nursing actions and communicate with families, case managers, and physicians
• Coordinate supplies, medications, and feedings to ensure adequate availability
  
  Ensure patient needs meet physician instructions

• LPN/LPT/LVN degree with current unrestricted license in WV
• Valid driver’s license with acceptable driving record
• CPR professional certification
• 1+ years of nursing experience preferred
• Home health experience preferred
