Base pay range

$32.30/hr - $41.67/hr

Compensation is based on the total number of years of experience in previous clinical roles.

In this role, you'll care for patients in Wells, Adams, Grant & Blackford Counties
.

You will work 32 hours each week with weekend rotations

POSITION TITLE:

RN CASE MANAGER

FLSA STATUS: NON-EXEMPT

REPORTS TO:

Clinical Team Leader

POSITION OBJECTIVE:

Manages and coordinates the provision of nursing care for assigned terminally ill palliative care patients by assessing needs, implementing nursing care plans, making appropriate revisions, and evaluating outcomes of care.

QUALIFICATIONS:

  • Graduate of an accredited school of nursing.
  • Bachelor’s degree in Nursing preferred.
  • Must have a current RN license to practice in the state of Indiana.
  • Must hold current CPR certification or obtain within the orientation period.
  • Must be able to provide those services requiring substantial specialized nursing skill.
  • Prefer one (1) year of recent hospital or home health care experience.
  • Must have a current motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
  • Must be able to drive independently to all patient/family clients’ homes.
  • Must be able to safely perform the essential functions of the job without posing a direct threat to their health or to others in the workplace.
  • Must be comfortable working with hospice patients with an emphasis on palliative care.
  • Assists with in-service training of the community on the hospice philosophy as needed/requested.
  • Strong interpersonal skills.

ESSENTIAL FUNCTIONS:

  • Instructs LPNs, Home Health Aides, and/or CNAs on specific patient care procedures within approved policies.
