Speech Therapist Home Health Per Diem

Southwoods Health is hiring a Per Diem Home Health Speech Therapist.

• Perform patient-centered speech therapy evaluations and treatments that meet company, federal, state, and local standards, guidelines and regulations.
• Administer medically prescribed speech therapy treatment to patients suffering from injuries or disease to restore function, relieve pain and prevent disability.
• Review physician referral/orders, patient’s condition and medical records to determine speech therapy treatment needs.
• Develop and revise speech therapy treatment plans based on evaluation and continued monitoring of patient strength, motor development, sensory perception, functional capacity, respiratory and circulatory function, records, and response to treatment.
• Consult with physician and other practitioners to obtain additional patient information, suggest revisions in treatment program and integrate speech therapy treatment with other aspects of patient’s health care.
• Determine referral needs and make applicable referrals in a timely manner.
• Provide appropriate level of supervision for speech therapist students, other licensed personnel and unlicensed personnel according to state and federal requirements.
• Document patient care services in accordance with regulatory requirements and within time expectations set by facility standards.
• Provide education to patients and patient families as appropriate, including information on post-op recovery processes, physician protocols, treatment plan, and home exercise programs.
• Plan and conduct in-services or training programs on speech therapy and related topics for medical staff, students and/or community groups.
• Demonstrate flexibility in work schedule to meet patient scheduling needs.

• Graduate of an accredited speech therapy program.
• Active Ohio state licensure.
• Current AHA BLS certification.
• Experience in Home Health therapy (preferred).
• Excellent communication and interpersonal skills.
• Ability to work effectively as part of a multidisciplinary team.
