Responsibilities
  • Establish goals and strategies for meeting discharge or continuing care needs with the patient, in collaboration with interdisciplinary teams.
  • Perform and document complete patient assessments and initiate and maintain current nursing care plans for all assigned patients.
  • Identify patient learning needs to provide individualized and comprehensive teaching.
  • Administer medications in a safe manner consistent with state practice and hospital policies.
  • Serve as a point of contact for patient care coordination throughout hospital departments.
  • Act as a patient safety advocate by participating in ongoing quality improvement in the department.
  • Serve as a patient advocate in collaboration with spiritual care, palliative care, and ethics.

Licensing/Certification
  • Current RN license in the state you are working or covered by compact (required)
  • BLS (required or preferred upon hire; must obtain from approved source prior to direct patient care; ACLS acceptable in lieu of BLS)
  • RN-BC, CCRN, PCCN, CMSRN, NIH Stroke Certification (preferred)
Education
  • Bachelor of Science in Nursing (BSN) preferred
Experience
  • One year of RN experience (required)
  • Six months RN experience in an acute care hospital setting (preferred)

  • Assigns patient care according to nursing skill, knowledge, and scope of practice
  • Educates nursing staff and others based on quality improvement indicators and outcomes
  • Clinical knowledge and competency to provide evidence-based patient care
  • Conducts comprehensive assessments and develops and prioritizes plans of care
  • Coordinates care across departments and with families; supports patient safety and quality improvement
  • Demonstrates strong interpersonal, verbal and written communication
  • Applies cultural diversity and inclusion principles
  • Seeks help when needed and engages in ongoing professional growth
  • Maintains professionalism and stewardship of resources
  • Adapts to stress, protects patient safety and privacy
  • Utilizes office and clinical technologies
