• Drive high-quality, safe, and cost-effective patient care in post-acute care communities
• Inpatient rounding in skilled nursing facility, inpatient rehabilitation facility, and long-term care hospital
• Work closely with hospice, palliative care, and home care agencies for safe patient transitions
• Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines
Compensation & Benefits
• Compensation is a productivity-based model (Fee for Service). Expected compensation is estimated income range of $235,000 to $278,000 annually with no cap on productivity bonus income potential.

Qualifications
• Board certified family medicine, internal medicine, or emergency medicine
• Willing to train experienced candidates as well
