Home Health Occupational Therapist –

The Home Health Occupational Therapist will provide occupational therapy services to residents and clients in their home environment to promote functional independence, safety, and quality of life. The role includes evaluating clients, developing individualized plans of care, implementing evidence‑based interventions, and collaborating with the interdisciplinary care team.
Program Features- In‑house program within a welcoming, slower‑paced community environment that delivers holistic care with a strong focus on prevention.
- Specialized on‑site training delivered during paid work hours, with all sessions eligible for CEUs.
- State‑of‑the‑art rehab gyms and equipment.
- Opportunity to build relationships by following residents across the full continuum of care, including inpatient, home health, outpatient, and fitness services.
- Clinically driven, patient‑centered treatment decisions guide all programs and care plans.
- Cross training provided to support all rehabilitation areas within our communities.
- Limited travel within and between our private communities.
- Student loan forgiveness available after 10 years (non‑profit).
- Provide occupational therapy services in home settings.
- Evaluate clients, develop individualized plans of care, and implement evidence‑based interventions.
- Collaborate with the interdisciplinary care team to achieve optimal outcomes in compliance with physician orders, state practice acts, and federal/state home health regulations.
- Master’s degree or Doctoral degree in Occupational Therapy from an accredited program.
- Minimum of 1‑2 years of experience in home health is preferred.
- Valid Occupational Therapist license or eligibility for license in the state(s) where services are rendered.

PayRange
$74,160.00 – $93,463.00 per year (starting rate varies based on skills and experience).
