Staff Pharmacist Rotate

Overview

The Clinical Pharmacist is responsible for providing pharmaceutical care to patients; supports drug-dose relationships with physicians and staff pharmacists; monitors drugs to ensure safe and effective use and to avoid toxicity; provides Nutritional Support functions. Participates in ongoing Performance Improvement programs and related clinical pharmacy practices per Joint Commission and ASHP guidelines.

• Provide pharmaceutical care to patients and support drug-dose relationships in collaboration with physicians and staff pharmacists.
• Monitor drug therapy to ensure safety and effectiveness and to minimize the potential for toxic reactions.
• Provide Nutritional Support functions as part of clinical pharmacy services.
• Participate in ongoing Performance Improvement programs and related clinical pharmacy practices in line with Joint Commission and ASHP guidelines.

• Education:
  
  Minimum BS in Pharmacy required; preferred Pharm

• Experience/
  
  Qualifications:
  
  At least one year of hospital-based experience as a Clinical Pharmacist preferred; good communication skills and ability to relate effectively to hospital staff, department heads, administration, and medical staff.
• Certification/Licensure:
  Current & valid Florida license as Registered Pharmacist.
