We are looking to recruit an enthusiastic Patient Flow Co‑ordinator within the Cornwall Onward Care Team, based in Carew House, Bodmin. This person requires previous experience in patient flow.

An exciting opportunity to support patient flow from an acute hospital working alongside an experienced team.

The aim is to achieve a collaborative approach to hospital discharge, ensuring that patients only stay in hospital for the time required to complete their acute treatment and, where necessary, experience timely and seamless transfer of care to another setting.

To assist with the efficient day‑to‑day running of the Patient Flow Team, the role will provide administrative and clerical support to Nursing, Cornwall Council staff and any personnel in Carew, supporting the Onward Care Team.

Main duties of the job
  • Ensure patient needs are addressed based on the care plan and coordinated in the most timely and effective way.
  • Act as a link, liaising with other agencies and departments to provide efficient and effective communication to expedite progress towards onward care.
  • Develop working relationships with ward teams to enable them to support and coordinate flow across the acute trust.
  • In collaboration with clinical teams, health and social care agencies, support the Integrated Transfer of Care Hub in planning and managing the daily flow through the acute trusts.
  • Contribute to follow‑up assessments, care planning, identification of resources and service delivery to facilitate the appropriate and timely discharge of patients.

Person Specification

Education / Qualification

  • Good level of education, including Maths and English
  • Knowledge of medical terminology
  • Previous NHS experience
  • NVQ 4 Business Administration or equivalent knowledge or experience

Skills and Aptitude

  • Able to communicate effectively with staff across different disciplines
  • Ability to work independently and on own initiative with guidance from an immediate supervisor
  • Ability to prioritise own work within prescribed Trust policies
  • Knowledge of data protection and confidentiality
