6) to deliver assessment and treatment to patients in the community, including homes and care settings across North Cornwall. The post is split 0.5 Homefirst and 0.5 Community Rehabilitation Team, aimed at improving patient experience and continuity of care.
You will work with Integrated Neighbourhood Teams to support independence, recovery, and wellbeing.
