We have an exciting opportunity for an experienced and motivated Advanced Clinical Practitioner to join our developing East Cornwall Community Hospital and Urgent Care Services in a unique peripatetic role
. Working across Liskeard, Launceston, Stratton and other East Cornwall community settings
, you will play a pivotal role in delivering high-quality, person-centred care to patients with acute and complex healthcare needs, whilst supporting the transformation of integrated community services.

This autonomous and highly rewarding role will see you working across Same Day Emergency Care (SDEC), Community Assessment and Treatment Units (CATUs), Virtual Wards, Urgent Treatment Centres and Community Hospital inpatient services
. You will utilise advanced assessment, diagnostic and prescribing skills to provide expert clinical leadership, support timely decision-making, and help patients receive the right care in the right place at the right time.

As an Advanced Clinical Practitioner (ACP), you will provide autonomous clinical care across Community Hospitals, Same Day Emergency Care (SDEC), Community Assessment and Treatment Units (CATUs), Virtual Wards and Urgent Treatment Services in East Cornwall. You will assess, diagnose, treat and manage patients with acute, chronic and complex conditions, independently managing admissions, investigations, prescribing and care planning within your scope of practice.
