General Summary And PurposeProvides advocacy, education, assessments and interventions to patients and families in the inpatient hospital setting to achieve an optimal level of care. Coordinates with medical staff and outside organizations to establish specific treatment plans and facilitate transition planning to achieve quality clinical outcomes.
Skills, Knowledge, Education And Experience- Master Social Worker (LMSW) required (Idaho or Oregon);
Licensed Clinical Social Worker (LCSW) preferred. All candidates must provide licensure or proof of application in process for the secondary state within 6 months from hire date. - Master’s Degree in Social Work required.
- Two years of experience in hospital setting or medical social work setting preferred.
- Experience in case management, addiction services, mental health, or population health management desired.
- Knows, understands, incorporates, and demonstrates the Organization's Mission, Vision, and Values in behaviors, practices, and decisions.
- Provides interventions for patients, families, and caregivers as appropriate (brief counseling, supportive therapy, crisis intervention, grief counseling, psychiatric placement/referral, alcohol/drug assessment and referral, and referral to community resources).
- Participates in care coordination activities for patients identified as needing social work or complex case management interventions.
- Works collaboratively with healthcare team members to efficiently allocate resources.
- Accurately documents assessments, plans, and interventions in the EMR.
- Coordinates mental health and/or substance abuse facility care.
- Follows established patient choice policy.
- Coordinates with physicians and other legal entities regarding guardian ships, involuntary psychiatric holds, power of attorney, and advanced directives.
- Collaborates with physicians and hospital staff through multidisciplinary case conferences.
- Provides weekend and holiday social work coverage.
- Participates in department staff meetings and committees as required.
- Demonstrates knowledge and skills to care for all assigned age groups (neonate, child, adolescent, adult, geriatric).
- Adheres to NASW code of ethics and State Licensure Board of ethics.
