Registered Nurse Home Health Case Manager provides skilled nursing care and coordinates individualized care plans for patients in their homes. This role supports interdisciplinary teams and ensures compliance with home health and Medicare regulations. The position is based in Boise, Idaho.
Key Responsibilities:Provide skilled nursing care in patients' homes, promoting safety, independence, and quality of life
Serve as primary RN Case Manager, coordinating overall care plans for assigned caseload and interdisciplinary team
Perform comprehensive assessments, evaluating physical, psychological, and social needs
Develop, implement, and update individualized care plans based on needs and physician orders
Complete accurate, timely documentation per agency, state, and federal standards, including OASIS assessments
Monitor patient progress, identify changes requiring intervention or physician notification
Educate patients and families on medications, disease processes, treatment plans, symptom management, and home care
Review medications, identifying potential concerns or discrepancies
Communicate effectively with patients, families, physicians, and other professionals
Participate in case conferences and coordination activities as needed
Promote continuity of care throughout the home health episode
Maintain compliance with home health, Medicare, state, and organizational standards while managing time effectively between patient homes and documentation
Current and unrestricted Registered Nurse RN license in Idaho or eligibility for Idaho licensure
Minimum 1 year of Home Health RN experience
Current BLS/CPR certification for healthcare providers
Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from accredited program
Valid driver's license and reliable transportation
Ability to maintain appropriate automobile insurance
Strong clinical assessment, communication, organization, and critical-thinking skills
Home health, Medicare, OASIS, and care coordination experience strongly valued
Skills:
Previous experience as a Home Health RN Case Manager
Experience completing OASIS assessments and documentation
Familiarity with Medicare home health regulations and requirements
Experience coordinating interdisciplinary patient care
Familiarity with home health electronic medical records and documentation systems
Ability to work independently while remaining connected with the clinical team
Compassionate, patient-focused approach to nursing care
You hold an active Idaho RN license and BLS certification. You have at least one year home health RN experience. You excel at OASIS documentation and interdisciplinary care coordination
