Health Care, Full Circle Health is looking for a Pediatrics Case Coordinator to join our team!
The Pediatric Case Coordinator coordinates comprehensive, family-centered care for children and adolescents with complex health and social needs. Working collaboratively with patients, families, providers, schools, and community partners, this role develops and manages individualized care plans, facilitates access to healthcare and community resources, addresses barriers to care, and supports care transitions to improve health outcomes, continuity of care, and overall well-being.
The Pediatric Case Coordinator is responsible for identifying, screening, assessing, planning, implementing, coordinating, monitoring, and evaluating the healthcare services provided to patients at Full Circle Health.
8:00am – 5:00pm, Monday
- Friday daytime shifts.

Education:
- A.D.N. or B.S.N. or LPN from accredited College of Nursing, or a Masters in Social work from an accredited university.
Experience:
- Minimum of two years of clinical practice; previous population health, case management or transitions of care experience preferred.
- Pediatrics experience preferred but not required
Licensure / Certification:
- Valid license to practice as a Registered Nurse (RN) or LPN or Master's Prepared Social Worker in the state of Idaho, current BCLS certification, and Certified Case Manager certification preferred.
