Senior Helpers of Bolingbrook and Southwest Suburbs is a trusted, non-medical home care agency dedicated to helping seniors remain safe, independent, and comfortable  partner closely with hospitals, rehab centers, and healthcare professionals to ensure smooth, timely transitions from facility to home.

Position Summary

We are seeking a motivated and relationship-driven Hospital & Rehab Liaison to build and maintain referral partnerships with hospitals, skilled nursing facilities, rehab centers, and case management teams. This role is key to ensuring patients receive high-quality, non-medical home care support upon discharge.

Key Responsibilities
  • Build and maintain strong relationships with hospital and rehab discharge planners, case managers, and social workers
  • Educate referral partners on Senior Helpers’ non-medical home care services
  • Leverage existing hospital and rehab contacts to expand referral partnerships
  • Serve as the primary point of contact for facility referrals
  • Coordinate with internal care teams to ensure smooth transitions from facility to home
  • Attend care conferences, networking events, and community outreach activities
  • Track referral activity and follow up consistently with partners
  • Represent Senior Helpers professionally and compassionately in all settings
Qualifications
  • Experience in healthcare sales, liaison, discharge planning, or home care preferred
  • Strong interpersonal, communication, and relationship-building skills
  • Knowledge of hospital, rehab, or senior care environments is highly desirable
  • Established relationships and existing contacts in hospitals, rehab centers, SNFs, or case management departments strongly preferred
  • Self-motivated with the ability to work independently
  • Reliable transportation and willingness to travel locally
  • Passion for senior care and helping families navigate care transitions
