Behavioral Assessment Team Member - Social Worker

Citizens Memorial Hospital (CMH) is recruiting a LPC, LCSW, or Licensed Psychologist for our Behavioral Assessment Team (BAT) at Bolivar Family Care Center in Bolivar, MO.CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, rehab therapies and more, CMH has its community covered!

About the department: Supporting hospital and emergency room patients of all ages, the Social Services Department is available 24/7 to offer assistance with discharge planning, resource referrals, and consultative services. Working as part of an interdisciplinary team, Social Workers and Designees provide clear communications to patients, family members, and other care team members to ensure smooth transitions and continuum of care.

A typical day will include: Conducting safety risk assessments to make appropriate recommendations and referrals for safety planning with patients and/or caregivers.

• Review patient charts and consult the on-staff physicians regarding clinical concerns
• Meetface-to-face with patients to conduct diagnostic assessments to include crisis evaluation using appropriate risk assessment measures
• Provide resource referrals
• Document appropriate information with accuracy

This position is: On-call- PRN nights,required to present within an hour of call.

Job Requirements:

• Education:
  
  Masters Degree in Social Workor Masters/Doctorate in Psychology
• Experience:
  
  1-3 years behavioral health or substance abuse counseling (may include pre-degree practicum)
• Skills:
  
  Strong written and verbal interpersonal communication, time-management
• Licensure/Certification:
  
  Current Missouri LCSW, LPC, or Licensed Psychologist. We are unable to consider PLPCs at this time.
• Physical:
  Sitting for prolonged periods of time, lifting up to 20lbs on an infrequent basis
