Find your calling at Mercy! As a Nurse Navigator, you will play a pivotal role in leading and coordinating patient care across both inpatient and outpatient settings. Your mission? To ensure that every patient receives high-quality, timely, and safe treatment.
Your ImpactIn this dynamic position, you will:
- Collaborate with patients, families, physicians, and interdisciplinary teams to manage care plans and discharge planning.
- Promote professional standards while ensuring patients receive timely, safe, and effective care.
- Assess individual patient care requirements and make informed recommendations based on severity of illness and service intensity.
- Hold direct and indirect care providers accountable for their professional responsibilities and adherence to regulatory guidelines.
- Facilitate a seamless care model, focusing on quality patient care, safety, and financial components.
- Coordinate patient discharge planning and follow-up care, ensuring a smooth transition to post-acute services.
Location: Mercy Clinic Springfield - Gastroenterology Whiteside
Schedule: Full-Time, Monday - Friday
QualificationsWe’re looking for someone who meets the following criteria:
- Experience: Minimum of 2 years in a related field.
- Required
Education:
Graduate from an accredited nursing program. - Preferred Education: Bachelor of Science in Nursing.
- Certifications: Must hold an RN license in the state of practice and obtain BLS certification within 30 days of hire.
