Make a difference in senior care with CANES Community Care as a Nurse Practitioner. This part-time role emphasizes comprehensive assessments and transitional care for older adults, ensuring their safety and independence.
The Nurse Practitioner will provide primary care for community clients, focusing on physical, mental, and functional evaluations. You will work closely with clients, caregivers, and families to create actionable care plans. Your expertise in diagnosing and managing acute and chronic conditions will play a crucial role in avoiding hospital readmissions.
Key Responsibilities:
• Conduct comprehensive health assessments for older adults
• Manage acute and chronic health conditions effectively
• Collaborate on personalized client care plans
• Document assessments and interventions accurately
• Provide health education and promote self-management
Requirements:
• Registered Nurse (Extended Class) in Ontario
• Completion of an accredited Nurse Practitioner program
• Minimum 2-3 years recent nursing experience
• Background in geriatric or transitional care preferred
• Valid driver’s license and reliable vehicle
