Part-Time Nurse Practitioner with CANES

Location: Bolton

Join CANES Community Care as a part-time Nurse Practitioner focused on enhancing the lives of seniors. This role combines clinical practice with patient engagement to drive optimal health in community settings.

As the Nurse Practitioner, you'll be responsible for performing thorough evaluations and developing care plans for older adults transitioning from hospital to home. The position requires a commitment to maintaining quality care and safety standards. Your clinical expertise will help ensure that all clients receive timely support and are actively involved in their care.

Key Responsibilities:

• Perform health assessments and manage client health needs

• Facilitate transitions from hospital to home care

• Ensure accurate documentation and communication

• Provide educational support to clients and families

• Engage in quality improvement initiatives for care delivery

Requirements:

• RN (Extended Class) with College of Nurses of Ontario

• Accredited Nurse Practitioner program completed

• 2-3 years clinical experience as a Nurse Practitioner

• Experience in transitional or geriatric care preferred

• Familiarity with community health resources
