Overview

This is a new senior practitioner post created within the Bolton Specialist CMHT to enhance support offered to service users with a severe mental illness (SMI) and a co-occurring condition. The Senior Practitioner will lead a cluster team within the CMHT in line with the Community Transformation model to provide a flexible and proactive approach to supporting this client group.

The role will provide assessment, formulation, brief and extended interventions, and collaborative care planning with the multidisciplinary team (MDT) and wider substance misuse services. The post aims to strengthen pathways into substance misuse support and improve continuity of care between inpatient and community teams, and between the specialist CMHT and external providers.

Responsibilities
  • Deliver and embed training; support CMHT key workers; lead the co-occurring conditions cluster within the community transformation model; and hold a small caseload or undertake joint work.
  • Contribute to relevant Rapid Reviews of Care and facilitate reflective supervision groups.
  • Ensure people are not excluded from mental health or substance misuse services due to co-occurring conditions.
  • Maintain strong multidisciplinary relationships.
  • Cover the Bolton borough and travel as required.
  • Join the operational on-site/on-call rota and deputise for team managers as required.
Requirements/Experience
  • Experience of leading a team within a CMHT or equivalent setting.
  • Ability to provide assessment, formulation, and brief/extended interventions; experience in collaborative care planning with MDTs and external providers.
  • Willingness to undertake training delivery and supervision facilitation.
  • Flexibility to travel across Bolton and participate in on-site/on-call duties.
  • Commitment to reducing barriers to access for people with co-occurring conditions and evidence of strong multidisciplinary collaboration.
