Bolton Council’s Intermediate Care Team is seeking a Qualified Adults Social Worker to join their dedicated service. This is a fantastic opportunity to work in a fast-paced, supportive environment focused on strength-based practice and promoting independence.

The Role
Undertake assessments and reviews under the Care Act 2014
Complete support planning and connect individuals to universal and voluntary services
Support complex assessments, including Mental Capacity and Best Interest assessments
Work collaboratively within a multi-disciplinary team
Build and maintain strong relationships with service users and carers Requirements
Diploma/Degree in Social Work
Minimum 3 years’ reference history (gaps in employment checked)
Relevant qualifications in line with JDPS
Experience working with adults in a statutory setting
Strong assessment, communication and partnership-working skills
