Responsibilities of this role:

• Undertake Care Act Assessments for adults with a range of care and support needs.
• Develop, implement, and review person-centred support plans.
• Facilitate appropriate care arrangements and monitor outcomes.
• Use the Mental Capacity Act to support professional decision making.
• Identify, assess, and manage risk while promoting individual choice and independence.
• Recognise and respond appropriately to safeguarding concerns.
• Support and advocate for service users to ensure their views and wishes are represented.
• Prepare reports and documentation for decision-making forums and meetings.
• Work collaboratively with health professionals, partner agencies, and multidisciplinary teams.
• Support carers and promote their rights to assessment and support under the Care Act.
