Working with colleagues, partners and local services, you will take a person-centred approach to supporting customers, helping to create safe, inclusive and thriving communities.

Key Responsibilities
  • Provide advice, guidance and low-level support to older and vulnerable customers, helping them maintain independent living and sustain their tenancy.
  • Carry out assessments, identify support needs and develop practical solutions that help customers achieve positive outcomes.
  • Create, review and monitor support plans, making referrals to specialist services and partner agencies where appropriate.
  • Work collaboratively with internal teams, health and social care professionals, voluntary organisations and external partners to coordinate support.
  • Support tenancy sustainment by promoting early intervention, identifying risks and helping customers access financial, wellbeing and housing-related support.
  • Maintain accurate, confidential and up-to-date records, ensuring customer information is handled in line with data protection requirements.
  • Champion equality, diversity and inclusion in all aspects of service delivery while helping to identify and report safeguarding, health and safety or wellbeing concerns.
What you'll need to succeed
  • Experience of working with older people and/or vulnerable customers within a housing, support, care or community-based setting.
  • A caring, respectful and customer-focused approach, with the ability to build positive relationships and gain trust.
  • Strong communication and listening skills, with the confidence to have supportive conversations and explain information clearly.
  • Good organisational skills and the ability to manage competing priorities while maintaining attention to detail.
  • Knowledge of safeguarding responsibilities, confidentiality requirements and appropriate professional boundaries.
  • Strong administration and digital skills, including experience using Microsoft applications and maintaining accurate records.
  • A proactive and solution-focused approach, with a commitment to equality, diversity and inclusion and delivering positive outcomes for customers.
