• Provide one‑on‑one skilled nursing care to medically fragile patients in their homes.
• Maintain up‑to‑date charting and accurate documentation.
• Communicate effectively and respectfully with patients, families and other health‑care providers.
• Support patients in living at home, attending school, activities and social events.
• Operate as the face of Avail Home Health & Senior Care Management in every patient interaction.

• Must be licensed as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in Washington State.
• Current CPR certification required.
