Field Nurse (LPN) – Concerto Care

Concerto Care is seeking a Licensed Practical Nurse (LPN) to be a core member of a multi‑disciplinary Field Team that delivers home‑ and community‑based care to patients with complex needs. Work in your community, build relationships with patients, and leverage proprietary technology to provide comprehensive care.

• Partner with team members to develop and implement clinical treatment plans under supervision of providers and RNs.
• Support wrap‑around clinical services in the home and field, including co‑visits, assisted telehealth visits, and independent LPN visits.
• Provide clinical services and coordination as directed by a patient’s risk assessment and care plan.
• Conduct health screenings for newly enrolled patients and ongoing assessments.
• Perform wound care, phlebotomy, medication review, and patient status checks.
• Engage with patients who are unable to reach their homes to re‑engage them in the program.
• Cover planned and unplanned absences to ensure continuity of care.
• Collaborate with interdisciplinary team members to continuously improve quality, reduce costs, and enhance satisfaction.

• Valid licensed LPN in the state of employment.
• Experience providing holistic care, including care for patients with mental illness and substance use disorders.
• Current CPR and BLS certification.
• Proficient computer skills (Microsoft Word, Excel, Outlook).
• Ability to travel as needed; valid insurance and proof of liability required.
• Driver license for a minimum of 5 years with no recent major violations.

Concerto Care requires all frontline workers to be fully vaccinated and provide documentation. Medical or religious exemptions will be considered with documentation.

Base salary $35.00–$42.00 per hour with annual bonus. Compensation may vary based on qualifications.
